GP Subscription Scheme

Terms & Conditions

Opening Hours:

  • LycaHealth Canary Wharf is open from 8am – 8pm, Monday to Friday.
  • The clinic is not open on weekends or bank holidays.
  • GP service availability varies day to day.
  • Subscription does not guarantee same day GP availability.

Membership

  • Members are patients who pay an annual subscription to the centre for packaged access to GP consultations at a considerable discount compared to paying as you go.
  • All applications are assessed by our Lead GP to ensure fairness to all existing members and to not over-subscribe. If high numbers of applications are received, then we will suspend new application assessments until we have the capacity to take new members.
  • Lycahealth has the right to decline a membership application if they feel the patient has a complex medical history or condition that would be better managed by another healthcare professional, or within a hospital that is set up to manage more complex needs.
  • Members will be required to attend 10 minutes early on their first appointment booking to ensure registration and key information is noted – namely NHS GP name and practice, and to provide information on any/all medications by which they are currently on, at the time of completing the registration.
  • Individual and spousal/significant other memberships are available. The subscription rates are reviewed annually, and members will be informed in writing of any changes to subscription fee, services, or policies, before they become due for renewal.
  • Membership subscriptions can be paid annually for a minimum term of one year (12 months). Annual subscriptions can be paid by debit or with credit card only.
  • We do not take cash payments for membership purchase.
  • Elite membership cardholders are entitled to unlimited GP appointments, free private referrals, and 20% discount on onward diagnostic self-pay imaging costs (MRI, CT, and Ultrasound only), subject to our Fair use policy.

Core | Elite | ‘Significant Other’ Entitlements:

‘Core’ membership holders are entitled to:

  • 6 face-to-face GP appointments a year (12-month period from purchase)
  • Free repeat prescription referral
  • Free onward GP imaging referral
  • 10% discount on onward diagnostic imaging cost at Lycahealth Canary Wharf. (Applicable to self-funders only, for MRI and CT scans only.
  • All allowances and entitlements are subject to our Fair Use policy.

‘Elite’ membership holders are entitled to:

  • Unlimited face-to-face GP appointments a year (12-month period from purchase)
  • Free repeat prescription referral
  • Free onward GP imaging referral
  • 20% discount on onward diagnostic imaging cost at Lycahealth Canary Wharf. (Applicable to self-funders only, for MRI and CT and Ultrasound scans only
  • 20% Significant other discount on self-pay GP appointments
  • 20% Significant other discount on onward diagnostic imaging cost at Lycahealth Canary Wharf. (Applicable to self-funders only, for MRI, CT, & Ultrasound scans only)
  • Free onward pathology referral
  • Unlimited virtual GP consultations (subject to fair use policy)
  • Free onward referral for Breast Screening – (Subject to Breast screening referral criteria)
  • Elite member is entitled to 1x Essential (Basic) Health Check a year (12-month period from membership purchase)
  • Elite member can only nominate 1 (one) ‘Significant other’ named person
  • All allowances and entitlements are subject to our Fair Use policy.

‘Significant Other’ membership holders are entitled to:

  • 20% discount on self-pay GP appointments
  • 20% discount on self-pay diagnostic imaging cost at Lycahealth Canary Wharf
  • All allowances and entitlements are subject to our Fair Use policy.

Breast Screening referral criteria:

  • Breast screening eligibility will be determined against the following presenting criteria:
  • You are 40 and over.
  • You have no symptoms relating to your breast.
  • You have not had a mammogram within the last 12 months.
  • If all criteria are met, patient will be eligible for breast screening allowance.

Cooling off period:

  • When memberships are taken over the phone or in person, members are entitled to a 14-day cooling off period by which if they should change their mind, we will provide a full refund of cost of package.
  • If an appointment has been utilised within the first 14 day cooling off period, and the member opts to cancel the membership, we will offer a full refund with the full price cost of any appointments utilised, deducted from the refund amount.
  • All refunds will be issued back to the members original card which was initially used to purchase the membership. We will not offer an alternative form of refund, nor offset the refund value against any other service.

Lycahealth Fair Usage Policy:

Our fair usage policy is implemented on trust to ensure patients use our services within a reasonable and sensible limit.

We believe this policy will encourage our patients to be considerate in their use of our services by ensuring that they do not book unnecessary appointments, turn up without an appointment, continually cancel appointments at the last minute, or not turn up for appointments which then stops other patients from being able to see a GP.

Fair usage for Core Unlimited Virtual GP appointments is deemed as no more than 3 virtual (telephone/skype/Teams) consultations a month for the term of your membership.

Consultations must be to enquire about GP recommendations, and to seek advice, based on symptoms discussed. A Virtual consultation can not be used to diagnose or treat a patient who is suffering from a physical ailment.

Fair usage for Elite membership allowance of unlimited face to face GP appointments is depicted as 2 new appointments per month, per year – (12-month period from subscription purchase).

  • Termination
  • Members who decide to terminate their subscription following their first anniversary must inform the Clinic Manager in writing – (David Larke: David.larke@lycahealth.com), giving one month’s written notice.
  • No refunds are given if members do not make use of our services in their subscription year.

Cancellation policy

  • We require 24 hours’ notice for the cancellation of an appointment.
  • Missed appointments or non-attendance (DNA) are chargeable at our standard rate and noted on the patient’s account.

Communication and confidentiality

  • Members should inform the GP or Clinic Manager of any preference in the way we communicate with them.
  • We request that members do not email any medical information to the centre, to protect their medical safety and privacy.
  • All clinical correspondence should be done in person with the GP.

Regulation

  • We are fully registered with the Care Quality Commission.

General Data Protection Regulation 2018

  • Lycahealth adheres to the General Data Protection Regulation 2018 (GDPR) in relation to personal data.
  • Members agreeing to these terms are consenting to the processing of their personal data to allow us to carry out work on their behalf.
  • Under GDPR 2018 principles, the member has the right to request details of all their personal data held by Lycahealth. All requests must be made in writing and marked for the attention of:
