For those who don't have a referral letter or a form, but would like to arrange a private MRI scan, private CT scan, private Ultrasound or private X-Ray, you can arrange to speak with one of GPs by filling out the following self-referral forms
A mammograph is an excellent diagnostic tool for further investigation. We accept self-referrals for non-symptomatic patients aged 40 and above who haven't had any mammography tests within the last 12 months. If you are symptomatic, a mammograph will require a GP referral.
